Healing Care Hospice is seeking an exceptional Registered Nurse to serve as a Case Manager in the areas of Santa Clarita & surrounding cities. This is a terrific opportunity to work for a mission-oriented hospice agency that cares about its patients, and its employees. Despite the COVID 19 pandemic we are still in need of staffing.

Experience

  • 1 Year of Registered Nurse experience

Language

  • Bilingual (English / Spanish) (Preferred)

Must Haves

  • Registered Nurse License

Shift

  • Monday – Friday
  • 8am – 5pm / Full Time

As a Registered Nurse Case Manager (RNCM), you will have the following responsibilities:

  • Coordinating the care for the patients assigned to you.
  • Making regular visits to patient homes to assess patient’s physical, emotional, spiritual, and social needs.
  • Working collaboratively with the hospice physician and are supporting LVNs to carry out appropriate interventions.
  • Managing and supervising the combined efforts of home health aides, social workers, and chaplains.
  • Upholding high standards of compliance regarding documentation and patient care requirements.
  • Helping the agency to continuously increase quality of care. Competitive salary and benefits provided.

 

Hospice experience is a huge plus, but we are open to training and developing exceptional new RN’s who want to develop an expertise in hospice and palliative care.

Salary

  • $40.00 – $50.00 per hour
